SAGE Program: Registration Information

We assume that at this point you have reviewed our materials and are ready to begin the process for registering to be a participant in our SAGE Program. To help you with this process, here are the steps we recommend (if you haven't done them already):
  1. Read and/or download our SAGE Program Frequently Asked Questions Document.
  2. Download and read carefully our SAGE Program Agreement to Participate Document.
  3. Read through the instructions and explanations for the SAGE Program Registration Questionnaire. (These can be found just below.)
  4. Complete and submit the SAGE PROGRAM REGISTRATION QUESTIONNAIRE form below.
Once we have received your completed SAGE PROGRAM REGISTRATION QUESTIONNAIRE, we will contact you with instructions on how to complete the next steps in our registration process and begin accessing our many resources for planning your trip. All information provided by you on your questionnaire will be considered confidential and subject to the requirements and conditions of our Avivara Privacy Policy.

SAGE Program Registration Questionnaire Instructions
Please note that all fields with an "*" are required. Simply write N/A in those fields which do not apply to you. For all date fields, please write the date in the following format: Month written out, dd, yyyy.

Personal Information

The first part of the questionnaire gathers personal information that we will need to stay in contact with you as we go through the SAGE Registration process. The fields should be fairly self-explanatory.

Occupation/Education Information
These fields help us to know a little bit more about your skills and/or current school situation. Again, these fields should be self-explanatory.

Travel Information
Anticipated Dates of Travel: Please enter here the approximate date you expect to arrive in Guatemala and the approximate date you expect to depart from Guatemala.

Estimated Total Budget: Let us know in this section how much money you have budgeted for your entire trip. This will help us help you in planning an experience that is tailored to your financial resources.
 
Traveling alone or with a group?: This helps us know if you are part of group that we will be planning for, or whether we should develop individualized plans for you.
 
Level of Spanish Fluency: This helps us to know how much assistance you might need when you first arrive. Obviously, folks with a higher level of Spanish Fluency will be able to be find their way around Guatemala with much less help from us. Generally, about two years of high school Spanish will put you at a mid-beginning level when you are actually "on the ground" in a Spanish-speaking country.

Personal Preferences Questions
These questions help us to have a better idea of what type of experience you are looking for. Based on what you share with us, we can make suggestions for "best fit" volunteer, study and recreation activities.

Reason for coming to Guatemala:
Just let us know what are the primary reasons you would want to study and/or volunteer in Guatemala.

Previous Foreign Travel Experience: This helps us to know whether you are fairly new at foreign travel or are a seasoned veteran. Tell us where you have visited before and how long you stayed.

Academic Seminars Desired: Some participants in our program will also want to earn academic credit for their time here (or just learn a little something more about Guatemala and its history.) That can usually be done by combining some academic study with your volunteer experience. Let us know in this section if you are interested in taking any of the seminars listed in our SAGE FAQ's document.

Type of Volunteer Setting Desired: In this section we would like to know if there is a particular social service area you would be most interested in working in (i.e., education, health, human rights, environment, etc.)

Previous Volunteer Experience:
Let us know what other experiences you have had as a volunteer.
 
Your Skills or Work Experience: This helps us to know what particular skill sets you might bring to a volunteer assignment.
 
Preferred Housing Arrangements: Here you would let us know whether you prefer to stay in a Guatemalan homestay, a hotel, or whether you will want to make your own housing arrangements after you arrive.
 
Special Medical Needs or Concerns: From first hand experience we have found out how important it is to have at some basic medical information about volunteers and visitors coming to our program. Here we would like to know about any medical issues you might have, medications you are taking on a regular basis, and/or any other pertinent information that would be helpful for an emergency room doctor to know if you were unconscious. Again, this information will be kept strictly confidential and only made available to our Avivara staff in case of a medical emergency
 
Questions for Us: Let us know if there are any other questions or concerns that you might have.

Agreement to Participate
This is a critical field on the questionnaire. Basically, we want to make sure that folks know what they are getting into when they travel to a developing country, so we do require that all participants in our SAGE Program provide us with a signed copy of our Agreement to Participate document upon arrival in Guatemala. (If a participant in the program is under the age of 18, a parent or guardian's signature will also be required.) It is possible that some sections of the document may be revised based on religious grounds, but these are generally handled on a case by case basis.

Emergency Contact Information

Unforeseen things can happen when traveling in a developing country. This information will help us contact a family member in case of an emergency during your visit.
